Frequently Asked Questions about Case Management

What makes Riverside Health Fitness Center (RHFC) different than other fitness facilities?

RHFC is so much more than a fitness center! The facility, hospital owned and operated, was designed to meet the needs of adults who are interested in reducing or managing disease risks. We are one of only a "handful" of fitness facilities across the country to provide the invaluable service of Case Management.

What is Case Management?

The focus of Case Management is to help each member achieve the maximum state of wellness by helping that individual improve and/or make progress in lifestyle habits. This is accomplished with one-on-one coaching and mentoring. For most participants, it results in a healthier, happier you!

What is involved in Case Management?

Upon joining RHFC, each member receives:

What else can I expect from Case Management?

Because so many of our members are older than 40 years of age and do not have a history of exercising, RHFC provides assistance and/or appointments to ensure that each member becomes comfortable and is fully integrated within the facility. We also provide:

What is my commitment?

Case Management is what you make it out to be! It also depends on how quickly you want to reach your goals. Case Management is an on-going process so there is no limit to the number of appointments.

What if I just want to join RHFC to work out?

As a medically-based facility, we expect our members to complete a health risk assessment and to be followed by our team of health professionals. Most members who have been initially hesitant to enter Case Management have found the program to be very beneficial.

Why is so much emphasis placed on risk factors?

We know that 70% of Illinois' population has one or more risk factors for vascular disease. And, in Kankakee County, an unusually high proportion of residents are at risk for cardiovascular disease. Our goal is to move our members toward a healthier lifestyle.

What is the cost of Case Management?

There is no cost for Case Management over and above the reasonably priced membership fees! Part of Riverside HealthCare's mission is to improve the health of the community. Providing Case Management at no additional fees will help the hospital fulfill this mission and better serve the community.
